Here you will find frequently used Rotary Foundation Forms.  All forms are in Acrobat .pdf format.

 

Effective December 5, 2011 the Individual Donor Contribution Form and the Recognition O& Transfer Request Form have been combined into one form.  Discontinue use of the previous forms immediately.

 

Contribution Form & PHF Recognition Transfer Request Form (123-EN and 102-EN) - This double-sided form is used for contributions from one person, club or district and/or for transferring Recognition Points from one person to another.  The form includes reqeusts for recognition materials.

 

Multiple Donor Form - (094-EN) Used whenever one check is sent for several contributors

Memorial or Tribute or Gift Form - Used to submit contributions as gifts to honor a loved one or memorialize that person.  Tribute Gifts can be a perfect birthday, wedding, or holiday gift for the person who has everything.

 

Recognition Replacement Form - Used to request replacement of TRF recognition materials such as PHF medallion, lapel pins, etc.
